I agree to hold harmless and indemnify Care-A-Lot® Pet Resorts (also known as Care-A-Lot® Pet Supply) and its employees, invites, and guests from liability, claims, and damages, including reasonable attorneys' fees, for loss, injury, illness and/or death caused by my pet to persons, other pets or property. I further release Care-A-Lot® Pet Resorts and its employees from liability, claims and damages, including reasonable attorney's fees, for any illness, injury and/or death caused to my pet, unless resulting from gross negligence or willful misconduct.

I also understand and agree that in admitting my dog(s), Care-A-Lot® Pet Resorts has relied on my representation that my dog(s) is/are in good health and have not been ill with a communicable condition within the last 14 days, and have not harmed or shown aggressive or threatening behavior towards any person or any other dog. However, as at times this is a cage free environment and there
are inherent risks that I accept.

While my dog is in the care and custody of Care-A-Lot® Pet Resorts and I am unreachable in the event of an emergency, I hereby authorize Care-A-Lot® Pet Resorts, its agents, and/or representatives to seek immediate veterinary care for my dog.

I understand that all costs in connection therewith, including transportation, veterinary, medical, and otherwise, shall be my responsibility. I will be responsible to reimburse Care-A-Lot® any medical expenses.

I understand that it is my responsibility to keep my pets vaccine records up to date with Care-A-Lot® Pet Resorts.

I understand that Care-A-Lot® Pet Resorts is NOT responsible for any items that may be lost, damaged or eaten.

Although we carefully screen all applicants, occasionally we discover that this is not an appropriate environment for every dog. Care-A-Lot® Pet Resorts reserves the right to permanently remove a dog from daycare at any time.

I fully understand the financial terms for boarding/daycare and am aware of the pet abandonment statute in Virginia. I understand that legal action will be taken against any owner for abandonment or
failure to pay in full my financial obligation to Care-A-Lot® Pet Resorts.

Although the staff at Care-A-Lot® Pet Resorts closely monitor your pet, sometimes active play and roughhousing pose the possibility of injury. Additionally, certain illnesses and infections can be transmitted during boarding. Animals identified as infectious will be isolated to the extent possible. Common injuries associated with daycare group play include, but are not limited to: bruises, abrasions, punctures, intestinal ailments (vomiting or diarrhea), foot pad soreness, and loss of appetite, weight loss, coughing and skin irritations. These problems usually resolve with appropriate treatment.

Care-A-Lot® Pet Resorts has specific hours of operation in order to accommodate both boarding and daycare guests. We strive to provide early morning pick up times and late evening pick up times to avoid an additional night stay for your pet. Owner agrees to pay $10.00 for every 15 minutes for any
daycare or boarding dog picked up outside of normal business hours.

I certify that I have read and understand the rules and regulations set forth on the preceding pages and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all of the terms, conditions, and statements of this agreement.

I AM OFFERING Care-A-Lot® Pet Supply the use of my pet(s) and/or me to be used in Care-A-Lot® Pet Supply promotional material, catalogs, ads, multimedia (such as television and the press), video, CD, DVD, and/or may appear on the Internet via Care-A-Lot® Pet Supply's website. I UNDERSTAND THAT the Internet is not a secure medium; that any material posted on the Internet is accessible to anyone with a computer and suitable telephone link AND THAT while the Care-A-Lot® Pet Supply has made every effort to secure its website, it cannot absolutely guarantee privacy or control the access to its website.

I, the undersigned, HEREBY CONSENT my pet(s) and/or me to be photographed and I AUTHORIZE Care-A-Lot® Pet Supply, its employees, agents, or authorized representatives to use, reproduce, publish,
transmit, distribute and display the said photograph(s) in any advertisement, Web site or other material for promotional purposes. Images are the property of Care-A-Lot® Pet Supply. The use of all images remains royalty free indefinitely on a voluntary basis and is not work for hire.
